This form is for clients requesting a custom design service that is not listed on my website.

Before filling out this form, please ensure you have contacted me to discuss your project details and confirm that I can fulfill your request. Pricing is determined based on the scope of work and will be provided to you during our initial discussion. Once confirmed, you may proceed with completing this form and entering the agreed-upon price below. Forms submitted without prior approval will not be processed.

Enter the price we discussed for your project below. This must match the amount provided to you during our consultation.

The price entered above covers only the agreed-upon design service. This price does not include any additional requests or extra revisions beyond what was initially discussed. Each design service (excluding web design) includes up to three free revisions. Additional revisions or modifications will incur an extra fee. Please see policies for additional information.

All Special Request Design payments must be submitted through either Zelle or Venmo. Payment instructions will be emailed to you immediately after submitting this form. Your turnaround time will begin once payment is received.
